Nonprofit agency providing a variety of services for seniors, persons with disabilities, and their caregivers/families. For questions regarding services, please contact the Lafayette central office – as there are specialists on call M-F, 8 to 4:30 p.m. The Montgomery Co. office is the base for the in-home services case managers, and they are not always in the office. The Agency serves Montgomery County along with 7 other counties.

Services include:

  • Link-Age Information and Referral Services – A clearinghouse for questions about services for seniors and persons with disabilities – provides information about options for care, assessments, referrals for in-home care; maintains a large database of resources available and listings of home health agencies, nursing homes, hospice agencies, etc.
  • Care Coordination – Across Generations - Trained professional helps frail persons to remain independent by coordinating a variety of in-home services to meet their needs for a fee;
  • Equipment Loan Program – Canes, walkers, wheelchairs available for loan. Availability determined by donations.
  • Family Caregiver Program - To assist caregivers of older adults as well as grandparents/relatives raising children through education, support groups, respite and other services.
  • Fitness and Health Programs - Living a Healthy Life - Chronic Disease Management Classes, Enhance Fitness exercise program, A Matter of Balance (fall prevention classes). Call for more information about times and locations.
  • Home delivered meals
  • In-home services – Home Health services may be provided to you if you meet the qualifications for the funding sources, CHOICE, Medicaid Aged and Disabled Waiver or SSBG.  Eligibility for funded services is determined by your income/assets/disabilities – call the central office for more information;
  • Pre-admission screening for nursing home services – call the Lafayette office to apply for that service;
  • Recreational programs - Senior Bus Trips – 5 day trips for persons age 55 and older for minimal cost; Senior Games and Golden Games in June each year; Wacky Wednesdays at the Movies – a movie and snack for $1 one time a month from March to October; Senior Art Contest.
  • SHIP (Senior Health Insurance Program) counselors regarding Medicare
  • Transportation – provides vans, etc. for the other rural counties;
